  • Certified Birth Certificate Short - Certified copies of birth certificates that may be used for practically all legal purposes, including proof of citizenship, school enrollment, personal identification, passports, social security, and employment. Short-form birth certificates are the most commonly issued copies with all the basic information included. Birth certificates are available for births since October 1, 1907.
  • Certified Birth Certificate Long - Long-form birth certificates contain the same information as a short-form one, but include additional information, such as birth facility, mother’s residence, time of birth, birth weight, and crown-heel length.
  • Uncertified Birth Certificate - An uncertified copy of a birth certificate contains the same information as a certified one. However, it will be printed on plain white paper with a stamp indicating that it isn’t applicable for legal purposes.
