Only the following individuals may be issued a certified copy of birth certificates after providing appropriate proof of relationship:

The registrant (name on the birth certificate) if of legal age (16)

Spouse or child of the registrant

Parents on the birth record

Maternal grandparents

Grandchildren of registrant

Guardian (with complete legal documentation)

Designated attorney
